Forms to be completed for High Stakes Exams
SDRC can support in the completion of forms for students seeking accommodations for high stakes exams. Please submit completed requests to email@example.com. If an appointment/meeting is required, you will be notified. Please allow up to 10 work days for your documentation to be completed from the date of the request. To ensure this process is not delayed, please make sure to review that the required information, that you have provided SDRC with the information needed to support your request, complete your section of the forms, provide SDRC with the forms you are needing to be completed. Please note that UCR SDRC is not responsible for the timeline or approval of exam accommodations for non UCR exams.
