The Student Disability Resource Center (SDRC) is open during regular business hours, Monday-Friday, from 8:00am-5:00pm (except for University Holidays and Closures, and periodic department training/in-service events). Documentation Guidelines

Certificate of Disability Forms

These downloadable forms are to be completed by your treating licensed provider and returned to SDRC before accommodations can be determined.  Please email completed form to sdrc@ucr.edu


Accessible Parking

Forms to be completed for High Stakes Exams

SDRC can support in the completion of forms for students seeking accommodations for high stakes exams.  Please submit completed requests to sdrc@ucr.edu. If an appointment/meeting is required, you will be notified. Please allow up to 10 working days for your documentation to be completed from the date of the request. To ensure this process is not delayed, please make sure to review that you have provided SDRC with the information needed to support your request, completed your section of the forms, and provided SDRC with the forms you are needing to be completed. Please note that SDRC is not responsible for the timeline or approval of exam accommodations for non-UCR exams.

Please Note: Each testing agency has its own process and timeline, ranging from days to months. Preparation and planning are key to accessing accommodations in high stakes exams.
